本表格所收集的個人資料，將用以辦理申請學位事宜。本校可能會把這些資料披露予教育局或其他獲授權處理個人資料的學校，用以處理學位分配或其他有關教育的事宜。
你必須在本表格提供你的個人資料。如提供的資料不足，本校可能無法處理有關申請。
你有權按照《個人資料〈私隱〉條例》第18 和22條及附表一第6原則的規定，查閱及更改個人資料。查閱資料的權利，包括取得本表格/紀錄所載個人資料的副本。
已收集的個人資料，將於完成學生註冊程序後銷毀。

The personal data collected in this Application Form (“the data”) will be used for the purposes of processing your application (“the Application”) for the Discretionary Places. The data may be transferred to EDB or other authorized parties facilitating the allocation exercise or other education affairs. 
In making the Application, it is obligatory for you to supply our school with the data requested in this Application Form.  The consequence for you if you fail to supply such data is that our school will not be able to process the Application.
You have the right to request access to and correction of the data according to No.18, No.22 and the Schedule No.6 of the Personal Data (Privacy) Ordinance. The right also includes requesting for the copies of this Application Form and the record of the personal data in it.
Thereafter, your application together with all the materials provided will be disposed of.
